Help please - penis issues?

Hi, my penis head goes almost scaly and withered looking and it looks like dead skin has formed. This occurs after I have had an erection and the penis returns to being un erect or after I ejaculate. I was wondering if this could be caused by scratching to the penis head? I've already visited the doctors numerous times but they haven't given me a clear answer. I would very much appreciate any help and advice.
