Can I Collect Social Security Disability at age 59 in different country?

Someone wanted to ask me this question.... If I have a dual citizenship Poland/USA, and i am currently disabled, unable to work, can i collect social security disability in Poland? I am planning to go there for a year and come back, can I still collect money for disability?
